Hello. This is Smart Skin Clinic, an aesthetic dermatology practice.

In this article, we explain — from a specialist's perspective — the features of Lala Doctor (LHALA Doctor), a 5th-generation peel that takes a completely different approach from conventional "exfoliating" peels by nourishing the skin from within, along with how it differs from other peels and what to expect in terms of downtime.

What Is Lala Doctor? A 5th-Generation Medical Peel That Doesn't Strip the Skin

Lala Doctor is a state-of-the-art peel treatment developed in Korea, available exclusively through medical institutions.

Conventional peels have typically worked by using acids to "strip away" the dead skin cells on the skin's surface. Lala Doctor takes a different approach.

Instead of stripping the skin, it nourishes it.

Using patented technology, Lala Doctor protects the skin's barrier function while delivering active ingredients deep into the dermis, promoting the production of collagen and elastin. It's a skin-nurturing treatment that uses no scalpels or needles.

The 7 Key Ingredients in Lala Doctor

7 Ingredients of Lara Doctor

The secret behind Lala Doctor's high efficacy lies in its 7 carefully calculated ingredients.
Here's how each ingredient works on the skin.

LHA (Lipo-Hydroxy Acid) | Gentle Exfoliation

This is the core ingredient behind 4th- and 5th-generation peels.

Compared with the acids (AHA/BHA) used in conventional peels, LHA has a larger molecular size and a mildly acidic pH of 5.5. Because of its larger molecular size, it penetrates the skin slowly, resulting in less irritation. It gently removes only unnecessary dead skin cells while protecting the stratum corneum's barrier function.

INCUBE 501 (Growth Factors) & 701 (PLLA) | Rebuilding the Skin's Foundation

These are the ingredients that make Lala Doctor a true "medical-grade" treatment.

INCUBE 501 (KGF/EGF)

By absorbing and removing excess inflammatory substances, it calms skin inflammation and supports the recovery of damaged skin.

It also acts as a growth factor that promotes the proliferation of keratinocytes and fibroblasts — directly boosting skin "renewal" and the building blocks of firmness. As keratinocytes proliferate properly, the skin's barrier function is strengthened, while fibroblast proliferation helps generate firmness and elasticity.

INCUBE 701 (PLLA)

Manufactured using uniform spherical PLLA particle technology, this ingredient helps add firmness and thickness to the skin.

It's the same material used in dissolvable surgical sutures and thread-lift threads. As it slowly breaks down within the skin, it powerfully stimulates collagen production.

Lipids and H2Sol™ | Strengthening the Barrier and Antioxidant Support

Lipids

Phospholipids are a key component of the intercellular lipids and cell membranes that form the skin's barrier function.
Lipids form a protective film over the stratum corneum, helping prevent the dryness that can result from peeling.

H2Sol™

Made from natural ingredients using patented technology, H2Sol™ activates fibroblasts to promote the production of collagen and elastin, supporting skin regeneration. It's alkaline, and while protecting the skin's barrier function, it also helps dissolve dead skin cells and melanin.

Other Notable Ingredients (QPT™, Calcium, and More)

[QPT™] Derived from green laver (aonori), this ingredient supports antioxidant and brightening effects. It absorbs light energy and efficiently transmits and concentrates it into the skin tissue, amplifying the effects of light-based treatments.

[Calcium] Helps repair the skin's barrier function and calm inflammation, supporting firmness, elasticity, and the reduction of fine lines.

Why "5th Generation"? How It Differs From Conventional Peels

Lala Doctor is called a 5th-generation peel — but what exactly makes it "5th generation"?

The history of peels in aesthetic medicine is, in many ways, a history of reducing irritation and downtime. Below is a summary of the characteristics of 1st- through 5th-generation peels.

GenerationRepresentative ExampleFeatures
1st GenerationAHA / alpha hydroxy acids such as glycolic acid and lactic acidRemoves surface dead skin cells. Highly effective, but also highly irritating, often causing pain and redness.
2nd GenerationBHA / beta hydroxy acids such as salicylic acidReaches deep into pores but can be drying. Suited to acne-prone skin; redness is common.
3rd GenerationTCA / trichloroacetic acidReaches the dermis, but the strong acid causes noticeable irritation and longer downtime.
4th GenerationLHA (Lala Peel)Evolved into a low-irritation, "skin-nurturing" peel using LHA.
5th GenerationLHA (Lala Doctor)Retains the low irritation of Lala Peel while adding medical-grade ingredients (growth factors, PLLA) to enable dermal regeneration — achieving inflammation control, barrier reinforcement, and collagen production all at once.

Peels up through the 3rd generation stripped away dead skin cells, which meant strong irritation and downtime. Lala Peel and Lala Doctor overcome these drawbacks of conventional peels by building up and nurturing the skin instead.

Most conventional peeling agents are acidic.
Acidic formulas work by coagulating proteins to strip the skin, which makes them highly effective but also comes with downtime such as pain, redness, and peeling.

Lala Doctor, on the other hand, is alkaline (pH 13).
The alkaline formula gently softens the stratum corneum, creating conditions that allow active ingredients to penetrate more easily. Into this softened skin, the large-molecule, low-irritation LHA slowly penetrates deep into the skin. This makes it possible to activate the fibroblasts that generate collagen and elastin — without damaging the skin's surface.

Scientifically Proven: "High Efficacy Even at Low Concentrations"

Because Lala Doctor's main ingredient is the same as that used in Lala Peel (also offered at aesthetic salons), and because it's gentler than conventional peels, some may wonder whether it delivers enough of a peeling effect.

Rest assured — a study comparing the effects of 1st-generation peeling agents (glycolic acid) with the LHA used in Lala Peel and Lala Doctor found that LHA delivers strong results.

According to this research, LHA — the main ingredient in Lala Doctor — showed approximately 1.3 times greater improvement in wrinkles compared with glycolic acid peels, along with superior improvement in hyperpigmentation. And it achieves these results at a lower concentration than conventional peels.
This suggests that Lala Doctor may deliver strong anti-aging benefits while minimizing the burden on the skin.

The Difference Between Lala Doctor and Lala Peel

"What's the difference between Lala Peel and Lala Doctor?"This is one of the questions we hear most often.

Put simply, Lala Doctor is Lala Peel's"medical-institution-exclusive, high-performance version".

Ingredient Differences | Medical-Grade Exclusive "INCUBE" and Growth Factors

  • Lala Peel: Mainly focused on exfoliation care and epidermal renewal via LHA
  • Lala Doctor: In addition to LHA, contains "INCUBE," which includes growth factors (KGF/EGF) and PLLA — promoting regeneration from the skin's foundation, not just surface-level improvement

The decisive difference between Lala Peel and Lala Doctor lies in the concentration and range of ingredients used.

Lala Doctor is generously formulated with special ingredients — available only through medical institutions — in addition to Lala Peel's base ingredients. The most notable of these isthe patented ingredient known as "INCUBE".

Difference in Effects | Lifting Power and Skin Regeneration Capacity

These ingredient differences translate directly into differences in the depth of the effect.

  • Lala Peel's effects: Improved surface radiance, smoothness, clarity, and better makeup application
  • Lala Doctor's effects: All of Lala Peel's effects, plus increased collagen and elastin production in the dermis, which can help improve firmness, elasticity, fine lines, and skin tightening

In short, if your goal is simply a cleaner-looking skin surface, Lala Peel may be enough — but if you're looking for more comprehensive anti-aging care, such as tightening sagging pores or increasing skin density, Lala Doctor is by far the better choice.

Lala Doctor's Benefits | Addressing Pores, Acne, and Melasma

Lala Doctor does more than just remove surface dead skin cells.A treatment that elevates your skin's overall condition.

Improving enlarged, dark pores and controlling sebum

LHA, the main ingredient, is oil-soluble. This allows it to penetrate smoothly into sebum-clogged pores, dissolving away buildup and blackheads. Combined with the collagen production triggered by INCUBE in the dermis, it also helps tighten sagging pores caused by aging from within.

Improving and preventing acne and acne scars

LHA has antibacterial and anti-inflammatory properties. It suppresses the growth of acne-causing bacteria and calms existing inflammation. By normalizing sebum production and preventing clogged pores, it also helps create skin conditions less prone to breakouts. Because it's low-irritation, it can even be used on inflamed, acne-prone skin.

Lala Doctor's effectiveness for acne treatment has also been highly rated in medical comparative trials.

A study published in 2009 compared the effects of an LHA formulation with "5% benzoyl peroxide (BPO)," the global standard treatment for acne, in 80 patients with mild to moderate acne. After 12 weeks, inflammatory acne lesions decreased by 44% in the LHA group and 47% in the benzoyl peroxide group — statistically comparable, high levels of improvement.

Source:Bissonnette, R., Bolduc, C., Seité, S., Nigen, S., Provost, N., Maari, C., & Rougier, A. (2009). Randomized study comparing the efficacy and tolerance of a lipophilic hydroxy acid derivative of salicylic acid and 5% benzoyl peroxide in the treatment of facial acne vulgaris. Journal of Cosmetic Dermatology, 8(1), 19-23. doi:10.1111/j.1473-2165.2009.00418.x

Benzoyl peroxide is generally highly effective, but it also tends to cause side effects such as redness and dryness. These findings suggest that Lala Doctor (LHA) can be a gentle yet comparably effective option for those who find the side effects of standard acne medications hard to tolerate.

Improving Melasma and Dullness (Melanin-Dissolving Action)

Laser treatment can be effective for spots and dullness, but if melasma is present, the stimulation from lasers can actually make it worse.

H2Sol™, one of the ingredients in Lala Doctor, helps dissolve and clear melanin without irritating the skin. It also normalizes skin turnover, gradually pushing accumulated melanin pigment to the surface and out. This opens up new possibilities for melasma treatment.

Instant "Glass Skin" Glow and Radiance

Right after treatment, you'll notice your skin taking on a radiant, "glass skin" glow from within.
As old dead skin cells are cleared away and skin texture becomes more even, light reflects off the skin more beautifully — which is why it's a great option right before an important event.

Pros and Cons of Lala Doctor

BenefitsDrawbacks
Pain-free
No peeling
Suitable even for sensitive skin
Little to no downtime
Multiple sessions are needed for dramatic change

Lala Doctor is one of the few peel treatments suitable even for people with thin, sensitive, or atopic skin. Because you can apply makeup immediately afterward, it's also easy to fit in before work or other plans.

While you may notice a change in radiance and texture after just one session, achieving the full "skin-nurturing" effect that is Lala Doctor's core value — dermal remodeling — requires multiple sessions.

If your goal is to "remove a spot in one session" or "instantly eliminate deep wrinkles," laser treatment or injectable treatments may be better suited to your needs.

The Lala Doctor Treatment Process

STEP
Booking

You can easily book from your smartphone.

STEP
Reception

Check-in is "smart" and simple: just hold up your QR code at the reception desk.

STEP
Counseling & Examination

During the consultation, we'll check your current skin condition, listen to your concerns, and recommend the most suitable treatment. Please feel free to ask us anything — there's no pressure or pushy sales involved.

STEP
Treatment (Application of Lala Doctor)

The first solution (LHA) loosens dead skin cells, allowing the second solution (active ingredients such as INCUBE) to reach deep into the skin. Using a dedicated brush, the product is massaged in along the direction of lymphatic flow. There is little to no pain, so you can relax throughout the treatment.

STEP
Aftercare

We provide detailed guidance on caring for the treated area and precautions for daily life. From the standpoint of prevention and maintenance, we may also suggest appropriate home care, such as supplements, oral medications, and skincare cosmetics that can help address your concerns.

STEP
Going Home

What to Know on the Day of Treatment

  • Makeup: Can be applied immediately afterward.
  • Washing your face / bathing: Both are fine on the day of treatment, but it's best to avoid long baths, saunas, or anything that raises your body temperature, as this may cause redness.
  • Skincare: Your skin will be more prone to dryness and more absorbent after treatment. Moisturize thoroughly and be diligent about sun protection. Please avoid using retinol-containing products for 2–3 days after treatment.

Level of Pain During Treatment

Pain during treatment: Little to none

Some people may feel a slight warmth, tingling, or itching sensation as the solution penetrates the skin.
It is not painful enough to require anesthesia.

Downtime, Side Effects, and Risks

Downtime is minimal to none.

Because the treatment doesn't forcibly strip the skin's surface, you can return to your daily routine right away.

In rare cases, redness, itching, eczema, dryness, peeling, or acne flare-ups may occur, but these are temporary effects of the skin's regeneration process and typically resolve by the next day.

Even if you have an important event coming up, one of Lala Doctor's strengths is that it can be performed the day before or even the same day.

Who Should Not Receive Treatment (Contraindications) and Who Should Use Caution

  • Those who are pregnant or breastfeeding (safety has not been established; subject to physician judgment)
  • Those with wounds or severe dermatitis at the treatment site
  • Those with aspirin-induced asthma (caution required due to the salicylic acid content)
  • Those who have recently been sunburned
  • Those with inflammation or infection at the treatment site
  • Those with known drug allergies

If any of the above applies to you, please be sure to inform us during your pre-treatment consultation.
Also, please avoid using tretinoin, high-concentration vitamin A cosmetics, or peeling cosmetics for one week before treatment.

Treatments That Pair Well With Lala Doctor

Lala Doctor delivers excellent results on its own, but combining it with other treatments (combination therapy) can bring out its full potential. Here are some recommended combinations from our clinic.

Pairing With Injectable Treatments (Profhilo, Jalupro, Botulinum Toxin)

Lala Doctor conditions the epidermis and upper dermis, while injectable treatments target the deeper dermis and subcutaneous tissue. This "full-layer approach" enhances overall anti-aging results.

It pairs especially well with popular skin-nurturing injections such as Profhilo and Jalupro. Same-day treatment is possible — please feel free to ask us.

For more details on each of these treatments, please see the articles below.

Enhanced Penetration With Iontophoresis

We also recommend finishing your treatment with iontophoresis, which uses a mild electric current to deliver beauty ingredients deep into the skin.
With the skin made more receptive by Lala Doctor, iontophoresis further delivers active ingredients such as vitamins and growth factors, while its cooling, calming effect helps reduce redness.

Combination With Dermapen (Acne Scars and Pores)

Dermapen uses ultra-fine microneedles to create tiny channels in the skin. Combined with Lala Doctor, active ingredients can be delivered directly into the dermis through these channels.

Because this dramatically improves ingredient absorption, this combination is especially recommended for those dealing with crater-like acne scars or stubborn enlarged pores.

*This combination is also considered effective with other microneedling treatments such as Potenza.

Combination With Photofacial (IPL) *Jimbocho Clinic Only

Combining Lala Doctor with IPL (light-based) treatments such as Photofacial can enhance overall skin tone as well as help improve redness. Lala Doctor's anti-inflammatory action also helps calm any heat-related effects from the light treatment.

When performed on the same day as Lala Doctor, Lala Doctor is typically done first. With excess dead skin cells cleared away beforehand, light can reach deeper into the skin, enhancing the treatment's effect compared with either treatment alone.

*This combination is also recommended as a complementary therapy for those receiving laser treatment at other clinics.

Recommended Frequency and Expected Results Timeline

  • Treatment interval: Every 2–4 weeks × 5 sessions (1 course)
  • Maintenance: Once a month

[1st session] You'll notice improved radiance, clarity, and texture right after treatment. Effects last approximately 1–2 weeks.

[3rd–5th sessions] Recommended once every 2–4 weeks. Intensive treatment improves skin quality and makes it more resistant to skin issues.

[Maintenance] Continuing treatment about once a month helps maintain beautiful, youthful-looking skin.

FAQ

Can I see results from Lala Doctor after just one treatment?

Yes — many people notice a change in radiance, clarity, and texture (a smoother feel) after their very first session. That said, for pore tightening and overall skin quality improvement, we recommend a course of 3 or more sessions.

Does the skin peel afterward?

No — Lala Doctor is a "non-peeling" peel. Visible flaking generally does not occur. In very rare cases, slight peeling may occur, but it's mild enough to conceal with makeup.

Can I receive treatment while pregnant or breastfeeding?

To prioritize safety, our clinic does not perform this treatment on patients who are pregnant or breastfeeding.

I'm currently using retinol — can I still get Lala Doctor treatment?

If you're currently using retinol products, you may experience a stronger reaction after treatment. We recommend pausing use for about 3 days to 1 week before and after your treatment.

Can I get Lala Doctor treatment if I have melasma?

Yes, you can. Lala Doctor is low-irritation and also has a melanin-dissolving action, making it effective for melasma treatment as well.

Is it okay to use at-home Lala Doctor products or personally imported versions?

Lala Doctor is a medical-institution-exclusive formulation. Similar products are sometimes sold online, but their ingredient concentrations and quality control differ entirely from the genuine product. To avoid skin trouble, please be sure to receive treatment at a licensed medical institution.
