The underlying structure varies depending on the specific area in question.
A line that runs from the sides of the nose to the corners of the mouth. It becomes more pronounced as fat in the cheeks sags and pushes the skin outward. The assessment of the cause varies depending on whether the line appears only when smiling or remains visible even when the face is expressionless.
A line extending from the corners of the mouth toward the jaw. It results from a combination of loosening of the depressor anguli oris and orbicularis oris muscles, tension in the masseter and sternocleidomastoid muscles, and sagging of the skin around the jaw.

Muscle changes
When the depressor anguli oris and orbicularis oris muscles become lax, it becomes difficult to lift the corners of the mouth, and furrows form along the muscle borders. A well-developed masseter muscle and a weakened platysma muscle also contribute to sagging around the jawline.
Skin changes
As collagen and elastin in the dermis decrease and the skin’s firmness and ability to retain moisture decline, the skin sags and lines become permanent. Cumulative exposure to UV rays also plays a role.
Fat changes
When fat in the cheeks decreases or shifts downward, the upper part of the cheeks becomes sunken, and folds form in the lower part. This accentuates the shadows along the contours.

As we age, bone density in the upper and lower jaws decreases, reducing the foundation that supports the skin and fat. This is why results from superficial treatments tend to be short-lived—because these changes to the underlying structure remain.
