Whitening Internal Set

I've heard people say that they are taking the whitening set, and if it's good for the skin, I'd like to take it too. But I don't really know what a skin whitening set is."

I've been noticing more and more spots lately and it's starting to bother me..."

I've heard a lot about whitening sets, but I want to know if they really work."

There are many different whitening sets, but which one should I take?

Hello, I am a doctor at Smart Skin Clinic.

I am sure you are all asking yourself these questions.

We will address these questions from the perspective of a practicing cosmetic dermatologist in an easy-to-understand, medically-based manner.

Let us begin the explanation immediately.

What is a whitening set?

A whitening oral set is, simply put, "a set consisting of multiple drugs that have the effect of increasing the clarity of the skin.

To put it a little more medically, a whitening set is a set of medications with specific pharmacological actions that can prevent or treat dark spots and aging by taking them internally.

Each hospital may have a different set of offerings and a variety of plans.

But basically, tranexamic acid (transamine), vitamin C (Cynar), and vitamin E (Juvela),

It is often composed of drugs such as L-cysteine.

The baby's skin is the strongest, the most pulled and whitened skin.

This is lost over time due to aging and the accumulation of UV radiation.

Aging is an absolutely unavoidable aspect because of the genetic involvement. On the other hand, UV irritation can be prevented to a certain extent through individual efforts such as UV care and moisturizing.

Also, the whitening internal set, which has antioxidant properties, is useful against this UV stimulation, isn't it?

Why do spots form?

The substances causing the stains aremelaninIt is.

However, melanin itself is not a bad guy or anything.

It plays an important role in protecting skin cells from UV stimulation.

One of the most important ideas in medicine is homeostasis.

Simply put.A state of harmony without excess or deficiency is healthy. That is to say.

In the case of melanin, there is no problem if the following three points are in harmony

Factors that increase production: UV stimulation, excessive skin irritation, inflammatory response, stress

Factors that reduce production: Shading (UV care), inhibition of melanogenic pathways by oral medications

(3) Turnover: skin metabolism (decreases with age)

For example, even if children are outside in the hot sun and do not take any care of their skin, they will not be able to get rid of spots.

This is simply because (3) has a superior turnover function and metabolizes melanin before it overaccumulates.

On the other hand, the turnover function declines with age.

It is important to avoid the exacerbating factors in (1) and strengthen the inhibiting factors in (2).

If the balance of (1) - (3) is lost, spots will form.

Take a look at the following illustration. This shows that spots occur when turnover falls off.

Figure 1: Relationship between increased melanin production and turnover due to UV stimulation (adapted from Daiichi Sankyo)

This is the plan for a medically based whitening internal set.

Some of you are busy every day and pressed for time.

So, I will begin with a brief conclusion.

The following four types of products have been medically proven to be effective against dark spots and dark circles.

  • tranexamic acid

  • vitamin C
  • vitamin E
  • L-Cysteine

Tranexamic acid is the mainstay of oral treatment, and the following three products complement its effect.

What's your rationale?"

If you think so. It's smart!

I also believe that this is a very important viewpoint in an information society that is overflowing with a mixture of boulders and stones.

I often saw young nurses and nursing students being stuffed with this intense Word by senior nurses in the chief and master class in the wards.

(In fact, it is important to look not only at the evidence but also at the empirics. (In fact, it is important to have an empiric perspective as well as an evidence one, because if we were looking for evidence for everything, we would be stuck, as is typical in the early stages of COVID-19 and RNA vaccines).

If there is a treatment for which evidence has been established, it is wise to choose it.

Now, there are many clinical research studies to provide evidence, but the following is a list of the most famous ones.

  • Multicenter open study of DH4243 (oral tranexamic acid) for melasma (URL)
    • Conclusion:
In addition to vitamin C, L-cysteine, and pantothenic acid Ca, oral tranexamic acid was more than 30% more effective in improving the condition.
    • Subjects: 231 patients with melasma
    • Intervention details
      • A: 300 mg of vitamin C + 240 mg of L-cysteine + 24 mg of pantothenic acid Ca
      • B: A + tranexamic acid 750 mg
      • Duration: 8 weeks
      • Results (% improvement with skin tone color scale)
        • A: 26.5%
        • B: 60.5
      • Remarks:
        • The improvement rate was significantly higher in the 750 mg tranexamic acid addition group (group B) for all other evaluation criteria, including photographic judgment, imaging equipment, and patient satisfaction.

The result of our careful examination of numerous clinical trials and other papers is the set of four products mentioned above: tranexamic acid, vitamin C, vitamin E, and L-cysteine.

Each drug included in the whitening oral set and its characteristics

The following is a description of each of the drugs that make up the whitening oral set.

The oral set includes medications that prevent and treat blemishes as well as acne and anti-aging.

They consist of the following

  • Transamine tablets (tranexamic acid)
  • CINAR Tablet (Vitamin C)
  • Ubera Tablets (Vitamin E)
  • HI-CHIOL Tablets (L-Cysteine)
  • Glutathione Tablets (Glutathione)
  • Vitadans (vitamins B2 and B6)

The above medications work by inhibiting any of the pathways by which spots are produced.

The following diagram may be helpful.

It also demonstrates the effect of increasing the overall clarity of the skin.

Figure 2. Melanogenesis pathway and points of action of various drugs (taken from Daiichi Sankyo)

tranexamic acid

It exerts its anti-plasmin action (suppressing the substance plasmin, which accelerates the production of melanin), thereby inhibiting the pathway of melanin production and exerting an anti-inflammatory effect. (See Figure.2)

A rarely recognized risk is the formation of blood clots. For this reason, we do not recommend this procedure for patients with a history of thrombosis, pregnant women, pill users, or female hormone users.

Cynar tablets (a combination of vitamin C and pantothenic acid)

Inhibits melanin production and improves pigmentation by inhibiting tyrosinase activity and reducing dopaquinone.
In addition to its effectiveness in treating blemishes, it also has both antioxidant and collagen production-promoting effects.

Pantothenic acid helps vitamin C function and inhibits the secretion of excess sebum.

Since it is a water-soluble vitamin, overdosing is unlikely to cause major side effects, but it only increases the amount excreted in the urine, so it is reasonable to take it internally in moderate doses.

Ubera Tablets (Vitamin E)

Like vitamin C, it has antioxidant properties. However, when vitamin E exerts its antioxidant effect, it is oxidized and loses its potency
Vitamin C has the effect of restoring (activating) this deactivated antioxidant capacity of vitamin E. Vitamin C has the effect of restoring (activating) the antioxidant capacity of vitamin E that has been deactivated (see Figure 3). (See Figure 3).

By working synergistically with vitamin C, a higher improvement rate of spots and hyperpigmentation is observed.

Figure 3. Synergistic effect of antioxidant action of vitamin C and vitamin E (taken from Taisho Pharmaceutical)

HI-CHIOL Tablets (L-Cysteine)

Like vitamin C, it inhibits tyrosinase activity and suppresses melanin production.

It also has the effect of normalizing turnover (skin metabolism), smoothly expelling melanin, and lightening blotches.

Glutathione Tablets (Glutathione)

In addition to its effect of reducing melanin, which causes blemishes, it also has antioxidant and liver function improving properties. Therefore, it is often used as an anti-aging treatment.

Vitadans (vitamins B2 and B6)

It is a combination drug of vitamins B2 and B6.

It controls sebum secretion with vitamin B2 and promotes metabolism with vitamin B6. Therefore, it is effective as an acne treatment.

Three recommended sets

I know there are all kinds of medications out there, but in the end, what's best for me?"

I think it is natural to be so distressed.

In order to reduce such problems as much as possible, Smart Skin Clinic offers plans tailored to the problems and desires of each patient.

How did you like reading this?

The treatment of blemishes varies depending on the cause and pathology.

The oral whitening set is considered to be a relatively safe treatment because it is systemic in nature and does not increase local concentration or irritation. (Even if some types of spots are ineffective, there is no significant exacerbation.)

However, it is not smart to keep spending money on ineffective or scarce.

We hope that we can help you find the best treatment for your needs.
