What is an exosome?
Exosomes are substances that occur naturally within the body. In medical terms, they are tiny cellular vesicles enclosed in a lipid bilayer membrane. They are actively secreted by a wide range of cells and play an important role in cell-to-cell communication.
Through this role in cell-to-cell communication, exosomes are understood to exert a range of effects, including anti-apoptotic, anti-inflammatory, neurogenic, and angiogenic actions. These effects, in turn, are thought to contribute to what is commonly referred to as anti-aging.
Exosomes are granule-like substances 50-150 nm (nanometers: one billionth of a meter) in diameter secreted by cells.
One key function of exosomes that has drawn attention is their role in transferring information between cells. Exosomes are believed to act as a communication tool between cells.

Types of exosomes (by origin)
Exosomes can be grouped into three main types, according to the three principal sites from which they are sourced.
These three types are as follows:
- Umbilical cord-derived
- Fat-derived
- Bone marrow– and dental pulp–derived
Effect
The effects that may be expected from an Exosome IV drip are wide-ranging, as shown in the image below. This is related to the fact that exosomes are involved in cell-to-cell communication and act to help activate cells, while also working throughout the entire body.

Exosomes are thought to potentially influence causative agents of Alzheimer's-type dementia, such as amyloid-β and tau proteins, and are hoped to have a beneficial effect on cognitive function.
Reduced downtime
Exosomes are carried throughout the body via the bloodstream and move freely between tissues, exerting anti-inflammatory, antioxidant, and cell-repair actions through processes such as angiogenesis.
Through this mechanism, they may help accelerate wound healing and reduce downtime.
Effects on the hair
Just as they act on the skin, exosomes may act on hair—which is also made up of "cells"—and may help improve elasticity and body, and even support the appearance of gray hair.
Effects on immunity and allergic conditions
As noted in the medical paper below, exosomes may contribute to strengthening immunity.
These results indicate that circulating exosomes containing spike proteins play an important role in effective immunization following mRNA-based vaccination. This is further supported by the induction of humoral and cellular immune responses in mice immunized with exosomes carrying spike proteins.

As for the evidence behind the Exosome IV drip, a wide range of research is underway, as cited in the section on its effects.
Clinical research is still ongoing into the treatment of various conditions, including stroke, liver cirrhosis, cancer, lymphedema, joint disorders, and dementia.
Who cannot receive this treatment
As a general rule, those who fall under any of the following categories are not eligible to receive the Exosome IV drip.
In addition, please rest assured that a physician will examine you before the treatment.
- Those who may wish to donate blood or receive a transfusion in the future (once you have received the Exosome IV drip, you will no longer be able to donate blood or receive a transfusion)
- Those who have had an allergic reaction to an exosome-related treatment in the past
- Those currently affected by an infectious disease or in an immunosuppressed state
- Those who are pregnant or breastfeeding
- Those with a history of cancer or who currently have cancer (due to the possible risk of exosomes promoting cancer growth)
- Those with a blood disorder such as a coagulation abnormality, or who are taking anticoagulants
- Those with an autoimmune disease or a condition related to the immune system

Risks and Side Effects
Common risks and side effects that may occur with the Exosome IV drip include the following:
- Pain at the puncture site, bruising, and nerve irritation
- Other symptoms such as headache or nausea
- In rare cases, an anaphylactic reaction (an acute allergic reaction) may occur
There are also two points particular to exosomes: their potential impact on cancer, and their implications for blood donation.
Potential risk of promoting cancer
It has been suggested that, where cancer cells are present, exosomes may have the potential to promote the growth of those cancer cells.

On the other hand, there are also opposing research reports suggesting that exosomes may suppress the growth of cancer cells.

Use of umbilical cord–derived exosomes
At Smart Skin Clinic, we use "(1) umbilical cord–derived" exosomes—one of the three main types of exosomes: (1) umbilical cord–derived, (2) fat-derived, and (3) dental pulp– / bone marrow–derived.
Compared with other exosomes, umbilical cord–derived exosomes contain more than 14 times the amount of active components, as shown in the figure below.

We would also like to highlight two of these "active ingredients," including specific data.
(1) Rich in active ingredients for the skin
The first is data on the active ingredients for the "skin."
Exosomes offer a variety of anti-aging effects, one of which is their effect on the "skin." As the figure below shows, umbilical cord–derived exosomes contain a richer concentration of active ingredients for the skin than other exosomes.

(2) Rich in the rejuvenating protein "GDF-11"
The second is data on components involved in "rejuvenation."
There is a component called GDF-11 that is involved in rejuvenation, and umbilical cord–derived exosomes contain it in abundance, as the data below shows.

Exosomes vary in their extraction method, exosome size, storage method, shape, and so on.
For this reason, the International Society for Extracellular Vesicles (ISEV) has established guidelines for standardizing exosome protocols.
The guidelines proposed by ISEV are the "Minimal Information for Studies of Extracellular Vesicles (MISEV), 2018 Edition."

The Difference Between Exosome Infusions and Other Treatments
Even with the same exosomes, the purpose varies depending on “where they are delivered.” For whole-body anti-aging care, IV drips are recommended, while for targeted treatment of specific areas of concern, combining them with hydrafacials or Dermapen treatments is ideal. Dr. IshiiA single session can be effective. That said, the Exosome IV drip is a treatment designed to deliver more noticeable results when repeated. For this reason, we generally recommend a course of about 8 to 12 sessions, spaced roughly 1 to 2 weeks apart.
- Could it affect one's genes or similar?
Dr. IshiiTo date, no research has produced data showing that genetic information is altered.
- Could exosomes cause cancer?
Dr. IshiiAs a rule, no. Exosomes themselves are not thought to induce cancer. That said, if cancer is already present in the body, they may potentially activate it.
